Instructions for the CGU Health Examination for First-year Students
All first-year students (including dual-degree students) are required to have health examination on the registration day. Students may participate in the health examination arranged by CGU or arrange it by oneself at an approved public or private hospital in Taiwan. Students who fail to complete the health examination will not be able to collect the student ID card. Those who couldn’t attend CGU’s health examination at school must submit the report of health examination to the Health Care Section within 1 weeks after the semester begins.
Place: Indoor Stadium, 3F, Student Activity Center (behind the CGU Bus Station)
Fee: NT$1,000 (Instructions on print out the payment slip)
Notices before the Health Examination
Procedures:
Please report to the registration desk of Health Examination and bring the receipt of payment slip for verification.
Upon completion of the examination, please submit the first page of “Student Health Examination Form (will be distributed on site)” and the “Registration Procedures Form” to Health Care Section for confirmation and stamping.
Items of examination: the following items may be altered according to the health examination procedure of each examinee.
1. General Examination: Height, weight, pulse, blood pressure, visual acuity test, hearing, mouth cavity, and physical examinations
2. Blood Tests
3. Urine Tests
4. X-Ray Examination
5. Carbon monoxide examination
